Inquiry and Expression of Interest

The journey begins with you reaching out to us. This can be through an online inquiry form, a phone call, or an email expressing interest in our services.

Initial Contact

Once we receive your inquiry, a member of the Ubuntu Care Services team will contact you within 2 business days to gather more information about your needs and discuss how we can assist.

Pre-Assessment

We will review your NDIS plan and any other relevant information to ensure we can meet your needs. If required, we'll schedule a 'meet and greet' or phone interview to further understand your goals and preferences.

 Formal Referral

If you're referred to us by an external organization, like a healthcare professional or a local area coordinator, we'll touch base with them to understand your recommendations and requirements.

Service Agreement Proposal

We will draft a detailed service agreement that outlines the services we will provide, the cost and other terms and conditions. This ensures transparency and sets expectations.

Agreement Review and Finalization

You (and your support network, if applicable) will have the opportunity to review the service agreement. We encourage you to ask questions and suggest amendments if necessary.

Signing the Agreement

Once you are comfortable with the agreement, the final step is to sign it. This formalises the partnership between you and Ubuntu Care Services.

Start of Services

With the signed agreement in place, we will begin coordinating and delivering your services. Our team will ensure a smooth start and ongoing support.

The entire process from the initial inquiry to the commencement of services typically takes between 1-4 weeks, depending on the complexity of the services and the participant’s specific circumstances.
